We want to thank you for considering us for your dental needs! We are currently accepting new patients on a limited basis. There has been a heavy demand for new patient appointments, so we are currently operating with a waitlist. These requests are answered in the order in which they are received with priority given to emergency cases.

​Prior to your appointment, there is a new patient registration packet consisting of you medical history, our financial agreement, code of conduct, patient rights & HIPAA form, and our treatment consent form. In an effort to make things as smooth as possible for everyone, this packet is available for download below:

Simply print, fill them out, & bring them with you to your first appointment. These forms will always be available to you at our front desk as well. In that case, we simply ask that you arrive 15 minutes early to your appointment as to have you ready at your scheduled time.

IMPORTANT: Please alert the office if you have a medical condition or take certain medications that may be of concern prior to treatment. This information is important knowledge to ensure your overall health and may alter your  course of treatment. This includes:

​

• Artificial heart valves *

• Artificial joints *

• A pacemaker

• Use of any anticoagulant medications

​

*May require the use of prophylactic antibiotic treatment prior to dental procedures as ordered by your primary care physician or surgeon.

​

​If you require prophylactic antibiotics, Dr. Price will not fill any prescriptions until you are seen as an established patient in our office. You will have to contact your primary care physician or the provider whom requires the medication to have your prescription filled prior to your appointment.

While having your previous dental records from another dental office may be helpful in some cases, they are by no means necessary. Sometimes tracking them down can be tricky and time consuming. In an effort to save you the hassle, we do not require them. If you happen to have them in your possession at the time of your appointment with us, we will absolutely review them and add them to your account. If you wish to obtain them from your former provider and they require authorization to release them, the form below is available for your convenience. Your records can then be faxed, emailed, or mailed to the contact information provided on the form itself.
