For your convenience & peace of mind, we have made all of our forms readily available for download for your upcoming appointment or viewing purposes. Before any procedure, Dr. Price will thoroughly discuss any and all possible treatment plans. This includes the possible benefits, adverse events, or outcomes of the procedures. We know having any medical procedure done can be daunting. That's why making your treatment plan as least intimidating as possible is one of Dr. Price's main goals.
